GENERAL REQUIREMENTS

1. Citizens of the Republic of Indonesia
2. On March 1, 2013 age: D3 minimum 20 years maximum 25 years, S1 Education minimum 20 years maximum 27 years
3. Having a diploma issued by the State or private educational institution accredited at least B
4. Willing to follow the official schedule shift
5. Willing to live in Semarang
6. Grade Point Average (GPA): – Doctors, Nurses and Midwives min 3.0, – other Health Workers (formations other than the above) 2.75 min
7. Performance Criteria:
• a. Formation Midwives and Nurses Agency minimum height 155 cm (female) and 160 cm (men) with an ideal body weight / proportionate and look attractive
• b. Other formations minimum 150 cm (female) and 155 cm (men) with an ideal body weight / proportionate and look attractive
8. At the time of registering is not in a state of pregnancy
9. Good behavior, as evidenced by the Police SKCK valid
10. Passed in all stages of the selection includes administrative selection, selection performance, written test, PsikoTest, skill testing, interviews and Health Test.

REGISTRATION PROCEDURES AND EQUIPMENT REQUIREMENTS.

1. Candidates enrolled applicants by making application to the president of RSU Dr. Kariadi and files sent through the Post Office proposal to PO BOX 1104 Semarang
2. Deadlines may be no later than 22 March 2013 by Mark POS and files must be received by the Managing Committee no later than March 25 2013 at 12.00 hrs.

• g. Photocopying SIB / SIP / Pass Certificate of Competency Test (for midwives, nurses, pharmacy Technique Energy)

• i. For midwives formation must attach a certificate APN (Maternity Care Normal) and hope to have a certificate attached PONEK
